BDS Admission Checklist
Candidates admitted to the Bachelor of Dental Surgery (BDS) programme are required to produce the following original documents at the time of admission. Please ensure that all mandatory certificates are available to avoid any delay in completing the admission process.

| 16 | Undertaking for Liquidity Damages (₹500 Stamp Paper - CEE Format) |
| 17 | Passport Size Photographs (5 Copies) |

Important Instructions
- All original certificates must be produced during admission.
- Carry one set of self-attested photocopies of all documents.
- Reserved category candidates should produce valid supporting certificates.
- NRI candidates must submit all additional supporting documents at the time of admission.
- Anti-Ragging Undertakings should be completed online at www.antiragging.in and signed copies must be submitted by both the candidate and parent.
- The College reserves the right to request any additional documents required by KUHS, CEE Kerala or statutory authorities.
